Waiver & Informed Consent

Nature of the Practice
I understand that Zenthai Shiatsu is a holistic bodywork practice that may include mindful touch, pressure points, assisted stretching and joint mobilisation. I acknowledge that this practice is offered for wellbeing and relaxation purposes and is not a medical treatment, nor a substitute for medical, physiotherapy, chiropractic or mental health care.

Therapist Allocation
I understand that for this Open House event, the therapist providing my session will be allocated on the day. I acknowledge that all therapists are qualified and certified Zenthai Shiatsu practitioners and operate within their professional scope of practice.

Shared Room Format
I understand and consent to the fact that, while my session is a private one-to-one treatment, it will take place in a shared room alongside other sessions. I acknowledge that other clients and therapists may be present in the space and consent to receiving treatment in this format.

Health Disclosure & Communication
I confirm that the information I have provided at the time of booking is accurate to the best of my knowledge. I agree to inform the therapist of any changes to my health prior to or during the session.

I understand that I may communicate at any time if something feels uncomfortable and may request modifications, pauses or cessation of treatment.

Risks & Personal Responsibility
I understand that bodywork may result in mild and temporary effects such as soreness, fatigue or emotional release. I accept responsibility for my own wellbeing during and after the session.

Release of Liability
To the fullest extent permitted by Australian law, I agree to release and hold harmless the therapists, organisers and venue from any liability arising from my participation in this session, except in cases of proven negligence.

Cancellation & Refund Policy

A minimum of 48 hours’ notice is required to cancel or reschedule an Open House session in order to receive a full refund.

Cancellations made within 48 hours of the scheduled appointment time are non-refundable.

By booking a session, you acknowledge and agree to this cancellation policy.

Consent

This waiver is agreed to electronically at the time of booking.
